NorthCoast Health Care Management Services is an integrated network of providers for home health, home infusion, hospice, and population health management services. We provide comprehensive management of the patient’s health plan benefits for appropriate utilization of services for the patient and the health plan. Our goal is to obtain the highest patient outcomes in the most cost-efficient manner possible.
NorthCoast’s extensive panel of providers, all credentialed and contracted. With more a vast number of providers across the country, our network is comprised of hospital-based, not-for-profit, and for-profit, independent providers. They are local, regional and national providers dedicated to providing the highest quality care in the home and other alternate site settings. Our network provides services for Anthem Blue Cross Blue Shield and all other Blue Cross Blue Shield Plans, Anthem Medicare Advantage including members delegated to myNEXUS, Medical Mutual of Ohio, and other Health Plans.
